The nominals roles have to be updated again.
Respective I/Cs will collect all of platoon mates nominal roles, and send it to Azilah ma'am at raven_azilah@hotmail.com by Saturday 18092010,2359hrs.

The Nominal Roles MUST contain the following:(in order)
1. S/N ( according to alphabatical order)

6. Handphone Number
7. Date Of Birth
8. Home Address*
9.Email Address
10. Dietary Restrictions(What you cannot eat)
11. Medical History(Details of Last Relapse)
12. Drug Allergies
13. NOK(Next Of Kin) Name (Parents/Guardian)
14. NOK's NRIC
15. NOK relationship
16. NOK Contact Number
17. Remarks (leave it blank)
* No short Forms meaning, no BLK, write BLOCK
And the postal Code must be SINGAPORE ------
